Maxwell Elementary Middle School Athletics

Maxwell  Elementary & Middle School will now be a member of the AISA (Alabama Independent School Association). Therefore, each student-athlete must have Dr.'s physical examination along with each of the following forms listed below on file before the first practice.  Physical Examinations are to be turned in to the Nurse, all other documents should be turned in to the Coach.
